Job Description
The support worker will be responsible for supporting the individual with daily living activities in the home and community, developing interests in the community and driving the individual to weekly appointments and activities. The support worker will be responsible for supporting the development of skills and competencies that will assist the individual to pursue and fulfill their goals and aspirations.

Duties:
➢ Build a relationship with and get to know our family member by spending time with our family member
➢ Assist our family member to identify and participate in activities in the community, where they can connect with others around a common area of interest (i.e., recreational clubs, volunteer interests, associations). Involve our family member is creating plans for the week.
➢ Support and participate with our family member in participating at the Y and help him set up an exercise plan, participate in drop-in activities and swimming.
➢ Support our family member in exploring new activities/interests and support them to participate in activities, meet new people and develop friendships (i.e., hiking, basketball, soccer)
➢ Provide consistent and specific communication in direct, concrete, and non-abstract ways
➢ Spend time with our family member doing activities (e.g., board games, puzzles, movies,)
➢ Ensure our family member is appropriately dressed for the weather and activity
➢ Take our family member to weekly/monthly appointments
➢ Provide transportation to community involvement and appointments
➢ Be aware when problem solving is needed and support our family member to problem solve, when necessary, in a guiding and respectful way
➢ Respond to unpredictable situations, if necessary, in a calm manner and problem solve with team members. If you are unsure, check with family members.
➢ Support our family member by being present and answering questions respectfully by guiding and directing or redirecting
➢ Safeguard the individual in the home and community when needed

Communication
▪ Provide an update of the overall well-being of the individual including a brief summary of the week and any feedback from appointments.
▪ Keep brief records of successes and concerns

Accountabilities:
➢ Communication: Build strong and respectful relationships with the individual and their family. Demonstrate a strong understanding that the family is a valuable resource for troubleshooting any issues that may arise
➢ Provide a weekly update of the overall well-being of the individual including a brief summary of the week and any feedback from appointments in a communication mode agreed to with the family
➢ Communicate in a professional manner via emails or a medium discussed with the family in point form
➢ Confidentiality: maintain confidentiality at all times
➢ Interaction: Interact in a non-intrusive, non-threatening, image enhancing and age-appropriate manner
➢ Safety: Be aware of physical, social, and recreational environments, and taking action to minimize risks and maximize inclusion
➢ Documentation: Maintain any documentation as required by the family
➢ Advocate: Assist her to advocate for herself and advocate on her behalf when necessary.

Qualifications:
➢ Gentle, Creative, imaginative, knowledgeable, and self-directed
➢ Interest in sports and exercise an asset
➢ Knowledge of community resources available and ability to support the person’s participation and inclusion within their community
➢ Strong understanding of and commitment to inclusion
➢ The ability to problem-solve and deal with unpredictable situations
➢ The ability to demonstrate patience and use thoughtful language
➢ Coaching and mentorship skills. Lead by example and support our family member to continue learning new skills and develop increasing independence
➢ A valid Class 5 driver’s license; access to a reliable vehicle and applicable insurance and a clean driver’s abstract
➢ First Aid and CPR certification is an asset
➢ Recent Criminal Record Check within the last year
