Required Education, Credentials And Experience

LTC experience or experience with medication pass for 30 seniors; leading team of HCAs; wound and fall management.

Provide care education to patients in person or over the phone

CPR, ACLS, and BCLS Certification

Orientation for LTC provided, Acute (Med/Surg) + ER Experience

Recent acute care, emergency, and/or surgery experience.

1-2 years relevant experience preferred in Emergency, Med/Surgery.

Calls on Social Workers, Physicians and provides information regarding services offered by Care Advantage to include brochures and rates of services.

Makes initial contact with the patients and explains services offered.

Knowledge of OSHA, and Virginia State Licensure Regulations governing home health and nursing practice.

Delivering or supervising the delivery of skilled nursing services.

Reports progress of care to Social Workers who referred cases, as indicated.

Follows up and does start of care for referrals and other cases as assigned.

Demonstrates knowledge in the use of CPAP, BiPAP and Oxygen.

Communicates directly to Sleep Network Manager.

Demonstrates the ability to perform the essential functions as a Registered polysomnographer.

Maintains patient database with includes log of diagnostic procedures as they are performed.

Handles chemicals with appropriate safety precautions to self and others.

Maintains a neat and clean work area.

You are a team player with excellent communication, critical thinking and prioritization skills

Recent experience (within the last year) caring for inpatient Surgical patients is required

Excellent Physical Assessment skills are required

Working knowledge of computer programs is essential, knowledge of Meditech is an asset

Demonstrated organizational, communication and interpersonal skills – both written and verbal

Evidence of professional development and continuing education is essential

Evidence of ongoing professional development, continuing education and leadership experience

2 years experience and ability to work independently in NICU

NRP certification mandatory for NICU

Strong written, oral and time-management skills, including demonstrated experience delegating tasks to Personal Support Workers / Health Care Aides

Education and experience in gerontology, rehabilitation, community/complex/long term care an asset

Experience working with seniors and unregulated care providers

Strong assessment, documentation, communication and critical thinking skills

Current Basic Cardiac Life Support and First Aid certification

Preferably with 5 years of general Internal Medicine experience

Successful completion of Nephrology 1 or demonstrated nephrology experience in lieu of

Demonstrates knowledge of Peritoneal Dialysis (PD)

Demonstrated knowledge in Person Centred Care approach and principles

Good interpersonal and communication skills

Previous experience with cardiac arrhythmia management and associated devices preferred

Previous patient/staff education experience preferred

Current ACLS certification an asset

Critical Care or Coronary Care experience preferred

Previous Cath Lab experience preferred

Demonstrate knowledge of pacemakers and implantable cardioverter defibrillators

Current Certification with the Ontario College of Nurses

Recent clinical experience in a Cath Lab or equivalent setting an asset

Demonstrated commitment to continuing education within the past two (2) years

Previous experience in MSK/S.T.A.R. (A4) Rehab Programs preferred. Specialty certification an asset.

Effective verbal / written communication and interpersonal skills.

Diploma in Nursing Required, Bachelors of Science in Nursing (BScN.) or equivalent preferred.

Current registration with the College of Nurses of Ontario in good standing with no restrictions to practice
