Detailed Overview


In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person-centered and recovery oriented care, the Registered Nurse/Registered Psychiatric Nurse provides and coordinates the delivery of psychiatric nursing care services to clients who present with an age related mental health and/or substance use issue . The RN/RPN demonstrates nursing expertise and provides clinical leadership in evidence based practices through direct care activities including nursing assessments, crisis intervention, short term case management or referrals to other community services, and client and family education and health promotion activities. Sets priorities, and assigns and delegates tasks to support staff.


Responsibilities


  • Performs other related duties as required.
  • Provides consultation, support and informal education to families and other informal caregivers of the clients.
  • Communicates and consults with members of the health care team about clients' care; sets priorities, assigns and delegates tasks to support staff; participates in interprofessional conferences; explains plan of care to clients and their family.
  • Assesses client needs and available services and contributes to the shared care plan with the clients input. Facilitates clients' utilization of appropriate community services and rehabilitative programs through methods such as researching available services, liaising with the service and program providers and providing advice and consultation to colleagues.
  • Participates as an active member on nursing and hospital committees and councils for improvement of client care and/or work environment.
  • Provides collegial sharing of information on mental health issues to interprofessional shared care team in conjunction with other MH&SUS team members or with Psychiatrist, community groups and agencies; presents information/in-services to professional colleagues and other government/non-government agencies.
  • Screens referrals from the interprofessional team by gathering background information, determining service priorities and providing rapid response to urgent referrals, providing outreach, crisis intervention, and short-term treatment stabilization, and referral.
  • Carries out treatment plans and interventions, provision of support and crisis intervention, including facilitation of hospitalization where necessary.
  • Facilitates clients' utilization of appropriate community services by keeping knowledgeable about other services and liaising with them; collaborates with key agencies in order to ensure clients receive appropriate and timely service delivery and continuity of care.
  • Provides short term brief interventions, follows-up, monitors, and evaluates treatment outcomes.
  • Identifies gaps in service in order to offer more comprehensive care in the Physicians office.
  • Screens client referrals from the interprofessional team via methods which include gathering information from the referral source, client and other relevant persons, evaluating/screening information to identify priority of service required, assessing mental status and need for mental health services, hospitalization, or referral elsewhere and suitability of admission into specialized mental health groups and programs.
  • Participates in quality improvement activities and research by identifying client care issues and collecting data.
  • Writes timely and accurate reports of relevant assessments and observations including client and family education and evaluation of nursing care.
  • Provides referral to appropriate agencies and collaborative information sharing of psychiatric situations including substance abuse/dependence.


Qualifications


Education and Experience


Graduation from an approved School of Nursing. Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Valid Class V B.C. Driver's License and access to personal vehicle with appropriate business coverage.


Skills And Abilities


  • Ability to collaborate with and promote cooperation among members of the interprofessional care team.
  • Ability to use factual information and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
  • Ability to educate clients and families/care givers about topics essential to their health care and well-being.
  • Ability to operate related equipment including relevant computer applications.
  • Knowledge of mental illness and substance use treatment.
  • Knowledge of legislation and government policies and procedures that relate to mental health and substance use practices.
  • Knowledge of psychopharmacology (indications and side-effects).
  • Ability to promote person-centered care that demonstrates empathy for clients/families/care givers, sensitivity to diverse cultures and preferences, including advocacy for special populations.
  • Counselling and group facilitation skills.
  • Ability to communicate effectively verbally and in writing with clients, families/care givers, the public, medical staff and members of the interprofessional care team.
  • Physical ability to perform the duties of the position.
  • Ability to organize and prioritize work.
  • Ability to complete initial and ongoing mental health and substance use client assessment and provide nursing care through appropriate/prescribed technical, therapeutic, safety type interventions including crisis intervention.
