The Primary and Community Care Program Specialist is responsible for researching, developing, and supporting the implementation of leading practices and standardized clinic and Interprofessional Team based care business processes that enable effective and efficient primary care home service delivery models. The Primary and Community Care Program Specialist leads and promotes the integration of business processes within Northern Health in collaboration with clinical leaders, IT Clinical Informatics service and program managers and directors, physicians, and staff. The Primary and Community Care Program Specialist serves as the liaison with operational areas and accurately and promptly analyzes, prioritizes, and translates user needs to standardized processes.


This includes facilitating communication amongst end users, identifying and articulating gaps between operational business processes and systems, and facilitates the analysis, design, building, and maintenance of standardized processes. The Primary and Community Care Program Specialist is responsible for the overall direction, coordination, implementation, execution, control and completion of projects or initiatives for the Primary and Community Care program. Projects may involve process improvement and work redesign, productivity reviews, staff deployment and the provision of strategic information to support those processes. Working with stakeholders at all levels, the Primary and Community Care Program Specialist inspires others to achieve goals and deliverables through facilitation, effective communication of corporate vision, and ensuring the culture is one in which individual competencies can thrive.


Starting salary will be approximately from $80,935 to $101,168 and will be based on education, training, experience, and salaries of similar positions

Qualfications


  • Or an equivalent combination of education, training and experience.
  • Five (5) to seven (7) years recent, related experience in clinical operations and/or clinical systems facilitation.
  • Bachelor's degree in a health care or health informatics discipline.
  • Relevant professional development in areas of IM/IT, process improvement, change management, project management, group facilitation.

  • Strong documentation skills and demonstrated ability to develop detailed clinical or technical documentation, training aides and how-to documents, and track work activities or issues documentation.
  • Excellent interpersonal and communication skills; ability to foster collaborative partnerships using a team approach.
  • Strong organizational skills and experience in project management; ability to organize and prioritize work and balance multiple activities concurrently.
  • Ability to see connections between issues, events and data points and to think clearly around complex systems, bridging community issues and regional and provincial activities.
  • Ability to foster innovation throughout complex evolutionary change.
  • High level of motivation with a positive attitude and high degree of flexibility in a rapidly changing environment.
  • Proven ability to manage diverse groups and individuals through change initiatives and be able to resolve issues that arise throughout the process.
  • Demonstrated strategic thinking and analytical skills.
  • Strong computer skills and ability to work comfortably with spreadsheets, presentations, diagramming and word processing software (e.g., Microsoft Excel, PowerPoint, Visio and Word).
  • Ability and willingness to travel.
  • Physical ability to perform the duties of the position.
  • Ability to guide, coach, teach, and promote a learning environment and continuous quality improvement.
  • Superior small and large group facilitation skills and individual coaching skills.
  • Ability to work independently and as part of a team.
