Personal Support Worker (Psw) – Transitional Care Unit

PSWs are an integral part of a health care team, and in this role, you will encourage, support, and promote independence for patients in home and community care settings by assisting with activities of daily living under the supervision of our Personal Support Manager.

We are looking for a Community Support Worker (CSW) to work in our Transitional Care Unit. The work is within a safe community-based transitional environment where patients will be supported in achieving or maintaining optimum functioning while awaiting their next care destination either private retirement home, or long-term care. The program’s aim is to provide short-term transitional care with an estimated length of stay of 45 days.

We are hiring for a 4-month contract.

Full Time Shift work within a 12-hour model - 7am – 7pm.

• Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
