Job Description

This position will be responsible for all scheduling functions among all disciplines. This position can be based out of our NOVA (Herndon VA) office or our North Chesterfield/Midlothian office. This position can work regularly from either office or be a hybrid (office/home) position.

Essential Job Responsibilities
  • Places patients who are currently in hospitals or SNFs on old and run report daily.
  • Maintain equal distribution of visits amongst the clinician's ensuring productivity is met for all clinicians.
  • Assists with all scheduling processes from the Start of Care (SOC) through patient discharge.
  • Works closely with the Intake department daily to ensure excellent communication on referral volume and clinical triaging of schedules.
  • Other duties as assigned.
  • Aligns visits with clinicians in same geographic region, when possible, to reduce travel and improve capacity and reduce excessive mileage.
  • Uploads clinical documentation in the EMR system as needed.
  • Sends out schedules to clinicians on a daily basis at the end of each business day by 6 PM
  • Makes all schedule adjustments and ensure compliance with State and Federal regulations.
  • Daily follow up on all patients in facilities to ensure continued care needs.
  • Reschedules all missed visits inside of the Medicare week.

  • Adhere to all patient billing and documentation standards and policies
  • Function as a team member demonstrating collaboration with and responsibility to each patient, physician and office team member
  • Submit appropriate documentation based on Company requirements and timing utilizing the Company's current billing and Electronic Medical Records system.
  • Maintain an ongoing knowledge of community resources; effectively utilizing these resources to meet the patient's needs and to facilitate optimal outcomes throughout all care services provided
Requirements
  • Minimum of 2-3 years in a medical scheduling role; experience in Homecare Homebase (EMR) strongly preferred.
  • Ability to communicate effectively, both verbally and in writing, with all levels of an organization
  • Proficiency with reading and analyzing data.
  • Comfortable working collaboratively in cross-functional teams.
  • Highly organized with attention to detail.
  • Able to work flexible hours if needed to ensure schedules are completed and accurate.
Work Environment
To be effective in this role, you must be able to meet basic physical demands which include the ability to sit, stand, squat, twist and turn from time to time.
