Previous experience with phlebotomy is an asset

Assess and manage client care in the community, including palliative, wound, intravenous therapy and H/T chronic disease prevention

Comply with all standards, policies, procedures and legislative requirements

Current CPR and First Aid certification will be requested

Preference will be given to nurses who have palliative care experience and/or palliative care course

Experience working with pediatric and/or palliative clients

Preference given to nurses with Infusion and Central Line experience

Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.

Effective human relation skills and demonstrated commitment to working with seniors.

Effective communication skills and ability to work independently and collaboratively are essential.

Interprets and implements established policies, procedures, and regulations.

Plan, direct, supervise and evaluate the work of nursing and personal care staff.

Appropriate current certificate of competence from the College of Nurses of Ontario required.

Sound knowledge of nursing process, theory, and responsibilities to the nursing profession

Recent experience within the last five yearswith a minimum of two years of operating room experience preferred

Knowledge of Operating Room Nurses Association of Canadaof standards of practice required

Demonstrated excellent interpersonal, communication, problem-solving, conflict-resolution and decision-making skills required

Demonstrated knowledge of patient and family-centred care

Demonstrated experience with patient and family health teaching required

Recent experience within the last fiveyearswith a minimum of twoyears of operating room experience required

Knowledge of Operating Room Nurses Association of Canadaof standards of practice

Demonstrated knowledge of patient and family-centred care

Experience working with seniors and alongside Personal Support Workers, as well as education tied to gerontology, rehabilitation and long term care

Processing and administering medication in accordance with regulatory requirements;

Current basic Cardiac Life Support and First Aid certification

Proficient assessment, documentation and communication skills

Works collaboratively with Directing Registered Nurses, Health Care Technicians (PSWs), Management and patients.

Utilizes appropriate communication skills to obtain information for the client and/or family and appropriate members of the service delivery team.

Meets with individual team members to build education and learning plans and provides input to annual PDP.

Minimum 5 years’ experience in community based health care system.

Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;

Education and experience in gerontology, rehabilitation, community/complex/long term care an asset;

To facilitate and provide resident/family education;

To respond to and manage significant changes in resident's condition and or medical emergencies; provide emergency first aid;

Experience working in a senior's environment and with unregulated care providers preferred;

Must have demonstrated sound clinical knowledge;

Provide wound care, education, and medications to patients with general or straightforward illnesses or injuries

Use critical thinking skills to determine if a patient needs more specific or intense care

Minimum of 300 hours post clinical experience in a medical-surgical acute care setting within the past two (2) years

Proof of competency in medical-surgical skills

Current N95 Mask Fit Test Certification

Experienced in assessment of the newborn and transitional care required

Current experience with managing post-operative mothers and babies required

Experience with patient/family centred and/or partnership care models

Must have effective interpersonal, communication, organizational and problem solving skills

Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;

Education and experience in gerontology, rehabilitation, community/complex/long term care an asset;

Provides ongoing education to Associates to meet legislative requirements and learning needs of Associates,

Experience with the care of the elderly,

Good communication skills, both oral and written,

Provides direction and supervision to all PSW's/Care Attendants,

Assesses Residents on move-in and every six months thereafter unless there has been a change in the condition of the Resident,

Previous experience in providing and delivering patients and families with educational teaching

Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required

Minimum of 2 years acute care medical/surgical experience required

Recent experience with inpatient medicine, oncology and palliative patient care preferred

Demonstrates excellence in communication skills and interprofessional teamwork

Oncology Nursing – Registered Practical Nurse (Post-Graduate) preferred

Competent in organizational, time management skills

Demonstrated knowledge of good assessment skills

Minimum of one (1) year experience in home health, assisted living, hospital or long term care environment

Demonstrated knowledge of nursing practices, techniques and methods applied to heath and wellness resident care; federal and state and local regulations

Demonstrates good judgment, problem solving and decision making skills

Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications

Assisting in the management of the work of nursing and personal care staff.

Previous similar experience in long-term care an asset, with comprehensive knowledge of duties.

Excellent communication (verbal and written) and comprehension skills.

Developed creative problem solving skills to look at the “big picture”.

Ensuring exceptional resident care is provided while following protocols and maintaining physicians’ orders.

Observing, reporting and recording vital signs, symptoms and conditions of residents, and initiating remedial health measures where appropriate.

Assisting in the management of the work of nursing and personal care staff.

Previous similar experience in long-term care an asset, with comprehensive knowledge of duties.
