Demonstrated ability to use chronic disease management principles to empower patients to self-manage their conditions.

Use excellent problem solving and de-escalation skills to mediate issues and care concerns brought forward by patients, caregivers, or service providers.

2+ years of recent experience in community health or a related field.

Minimum Pay starting at $17.00 an hour for this location.

Ability to work well in a structured correctional environment.

Ability to read and follow company recipes.

Ability to lift up to 50 pounds.

Ability to lead, instruct, and assist with meal preparations including food preparation, baking, cooking, and clean-up.

High school diploma or equivalent

Minimum of three years of relevant planning experience in municipal planning with an emphasis on rural and small urban environments.

Working knowledge of the Planning Act and related senior government legislation and policies.

Demonstrated excellent computer skills, including the use of work processing, spreadsheet and database applications.

Experience working with Geographic Information Systems in a municipal government environment.

University degree in Planning or closely related discipline required, preferably at the Masters level.

Time Management: Balances a multitude of competing demands and prioritizes duties effectively.

Perform data entry into database

Administer and manage inbound and outbound mail, including priority post, packages, courier services, and other correspondence

Minimum (1) year of experience in a receptionist / administration role

Current certifications in Non-Violent Crisis Intervention (NVCI), Standard CPR / First Aid, and/or ASIST
