Assesses patients by physical examination, including pertinent diagnostic testing to determine health status.

Participates in the care planning process.

Supervises LPNs and certified nursing assistants.

Communicates with physicians regarding changes in resident's conditions, diagnostic test results, etc.

Documents assessments and care in compliance with standards of care and company policy.

Attends agency staff meetings, department staff meetings, educational offerings, and other offered hospice events.

Provides education, training, and support to the hospice patient/family as indicated.

Assists Intake team with daily operations including phone calls, referral tracking and general office duties.

Assists with Informational Visits and consent completion.

Supports and assists the Marketing Department in community events when needed.

Supports the compliance aspect of the Business Development department by collecting data, assisting with audits and compiling data.

One year of experience in an acute, hospice, home health setting, or long-term care setting.

Two years of experience preferred.

Graduate of an accredited School of Nursing

Must be currently licensed as an LPN in North Carolina and in good standings with NCBON

Ability to type 50 wpm preferred.

Working with the existing management and health services teams to promote communication and compliance to policies and procedures;

Valid CPR & First Aid Certification preferred

Previous working experience with elderly in long-term care or a senior living environment an asset

valid CPR & First Aid Certification preferred

Minimum of 3 years clinical experience or 2 years’ experience in medical case management

Must possess negotiation skills with insurance coverage and the ability to provide complete and timely case management reports as required

Comprehensive knowledge of worker’s compensation insurance and managed care highly preferred

Develop referrals from case managers, insurance companies and utilization review nurses

Strong oral and written communication skills required
