Geriatric Outreach Nurse Jobs

Knowledge of detoxification/withdrawal management, abstinence/harm reduction models of service for geriatric patients with substance use disorders.

Demonstrated ability to apply knowledge of theory and practice to a case management.

Three (3) year’s recent related experience working in geriatric mental health and addictions or an equivalent combination of education and experience.

Individual counseling and education and support to promote integration and continuity across the care continuum

Coordinates community resources and provides educational programs as required.

Share knowledge and expertise with colleagues via consultations, in-services and presentations.

Broad knowledge of crisis intervention and counseling skills.

Knowledge of the principles and practices of a client and family centered recovery model in mental illness.

Broad knowledge of mental health illness and treatment.

Broad knowledge of psychopharmacology (indication and side effects).

Broad knowledge of substance abuse and addictions treatment.

Broad knowledge of other facilities and community resources.

Certification in Reproductive Health (STIs) with the BCCNM is required.

Broad knowledge of nursing theory and practice within a client-centered approach.

Knowledge of Community development and health promotion principles and practices.

Broad knowledge of BCCNM Standards for Practice for registered nurses.

Broad knowledge of HIV/AIDS, STIs and communicable disease control.

Broad knowledge of mental health (including the Mental Health Act) and substance use including current pharmacological approaches to substance use treatment

Provides feedback to the Manager/delegate regarding the effectiveness of patient care programs and policy/clinical decision support tools and equipment.

Participates on nursing committees, care conferences and councils, as assigned or as agreed to with the Manager.

Comprehensive knowledge of nursing theory and evidence based practice within the designated clinical area.

Ability to role model evidenced based advanced practice skills in the designated clinical area.

Knowledge and experience in Trauma Informed Practice, Harm Reduction and Recovery Oriented Care.

Comprehensive knowledge of other health care disciplines and their role in client care.

Comprehensive knowledge of community nursing theory and practice within a client family-centred model of care.

Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity and professional judgment.
