Experience with Horizon Patient Folder/clinical portal/Meditech

Grade 12 plus completion of a Secretarial/Office Administration program

Knowledge and/or experience in dedicated case management or discharge planning

Demonstrated knowledge and/or experience in a community or long term care setting required

Minimum three (3) years' experience with the professional regulatory body

Knowledge of community agencies and resources

Excellent assessment, interviewing and negotiation skills

Excellent verbal and written communication skills

Demonstrates basic understanding of all Continuing Care Programs and a working knowledge of community services/agencies.

High school diploma and successful completion of a recognized secretarial/office administration program

Equivalent combination of training may be considered

Strong sense of responsibility and a high level of confidentiality

Proficiency in spreadsheet applications necessary

Demonstrates initiative and exercises good judgment to handle confidential information, correspondence and reports.
