2 - 5 years recent and related clinical experience

Previous experience in leadership roles

Under the direction of the Program Manager, coordinates inter-professional care plan development for each patient/client.

Under the direction of the Program Manager, coordinates episodes of care and supports discharge-planning processes.

Works collaboratively with the Program Manager to ensure effective utilization of unit based resources (staff and supplies)

Experience in patient scheduling/booking preferred. Experience with Community Wide Scheduling (CWS) an asset.

3+ years’ experience with Mental Health Crisis Management and/or Schizophrenia

Excellent assessment, risk management, and consultation skills

Provide practical hands-on work, when necessary, to demonstrate skill and assist in client development.

Understand and be able to integrate a philosophy of client-centred care into daily responsibilities
