Community Inclusion Advisor Jobs

Record the daily activity and personal health information (PHI) of the person we support on our case management database

Excellent understanding of behaviour management principles and strategies including mental health needs

Minimum 2 years of experience in community social services/health care

Ensure the supported person’s rights and choices are respected and advocated as needed

Interact effectively with people we support

Empower the person we support to be full citizens of their community

Plan, organize and deliver high quality safe and stimulating programs for individuals with disabilities

Experience working with individuals with a disability(ies) or an exceptionality(ies)

High Five Principles of Healthy Child Development or Sport Certification, considered an asset

Exceptional customer service and communication skills

Safeguard certification from an accredited organization, or ability to obtain within 1 month of date of hire.

Utilize specialized equipment and apparatus

Preferred formal education (Diploma or Degree, CSW, RCA, SIVA etc.) and/or work experience with persons with diverse-abilities and/or other relevant experience.

Provide support and advocate choice, while assisting individuals in their physical, social, emotional, recreational, economic, educational, and vocational life skill development.

Excellent interpersonal behavior and communication skills.

The desire and ability to work with persons with diverse-abilities and demonstrate personal suitability to promote dignity, independence, individuality.

Must be in good health with the physical ability to carry out the duties of the position and shift responsibilities.

Coordinate, implement, and facilitate participation in community-based social, recreational, and vocational activities- empowering individuals to be full citizens of community.

Analytical skills necessary for needs reviews, research, statistics, root cause analysis and planning

Ability to identify accountabilities, accomplishments and measurement methods to monitor progress toward goal attainment

Experience developing or applying diversity, equity and inclusion frameworks

Ability to use various computer software programs such as Word, Excel, asset management software and scheduling software.

Knowledge of needs assessment and strategic planning principles and practices, with demonstrated ability to implement sound community development processes.

Knowledge of participation processes, group facilitation, team building, and group dynamics.

Knowledge of inclusive and adapted program planning and practices.
