The salary range for this position is CAD $40.21 - $54.28 / hour

Detailed Overview


As a member of a multidisciplinary team, provides direct nursing care to patients of the Hospice Residence at assigned locations according to established policies and procedures by assessing physical, emotional, spiritual and psychosocial needs; plans, documents and implements care plans; evaluates the effectiveness of the care plan and makes recommendations for care plan changes, where necessary.


Responsibilities


  • Collects information about the patient using a variety of sources such as skills of observation, communication and physical assessment to enable appropriate nursing care.
  • Participates on nursing, hospital committees and councils for improvement of patient care and/or work environment.
  • Assesses, plans, implements and evaluates nursing care of assigned patients by assessing actual or potential problems and strengths, plans and performs interventions and evaluates outcomes.
  • Acts as an advocate to protect and promote the patient's rights to autonomy, respect, privacy, dignity and access to information.
  • Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor, where appropriate; identifies self-learning needs and attends educational programs to maintain and enhance clinical competency.
  • Maintains a safe environment for patients, families and staff by adhering to established safety and emergency practices and procedures.
  • Participates in quality improvement activities by identifying patient care issues and collecting data, evaluating patient care programs, techniques, standards and procedures by providing feedback about effectiveness and patient care outcomes.
  • Documents timely and accurate reports of relevant observations including patient and family teaching and evaluation of nursing care.
  • Communicates and consults with members of the health care team about the patient care plan; participates in interdisciplinary conference and explains plan of care to the patient and/or families.
  • Performs other related duties as assigned.


Qualifications


Education and Experience


Graduation of an approved School of Nursing and/or Psychiatric Nursing. Two (2) years' recent, related medical nursing experience in an acute care facility, including one (1) year recent related experience in a hospice/palliative care environment, or an equivalent combination of education, training and experience.


Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).


Skills And Abilities


  • Ability to work independently and as a member of a multidisciplinary team
  • Ability to operate related equipment including applicable software applications
  • Ability to develop and maintain rapport with the patient, family and caregivers
  • Knowledge of nursing practice as it pertains to hospice palliative/residence patients
  • Knowledge of other healthcare professions and their contribution to the multidisciplinary team
  • Physical ability to perform the duties of the position.
  • Ability to organize and prioritize work in a dynamic environment
  • Ability to communicate effectively both verbally and in writing
