Job Description
1.0 FTE (35 hours/week) - 2 Positions


Hamilton Urban Core Community Health Centre (HUCCHC) is a thriving inner city health centre and a leader in the provision of high-quality primary health services and health promotion programs. Our comprehensive health services are delivered through a dynamic team of multidisciplinary, inter-professional health providers. As an inner-city health centre our client populations include individuals and families with chronic and complex health and social conditions, many of which are living in poverty.


General Responsibilities: We are seeking Registered Nurses (RN) to join our Hamilton Urban Safer Supply Program (HUSS). The program provides clients using street-based drug supply, who are at high risk of overdose in the community and of medical complications due to drug use, a regulated supply of prescription through the Nurse Practitioner (NP) in an effort to reduce overdose and overdose deaths. HUSS is a multidisciplinary team that provides primary care and wrap around social supports to eligible safe supply clients. The team consists of Nurse Practitioners, Case Manager, Health Navigator, Cultural Ambassador, and Peer Mentor.


The RN will work within their full scope of practice in a harm reduction framework to provide assessment and treatment functions for HUSSP clients and work with the broader team in providing administrative and evaluation support for the program.


Specific Responsibilities


  • Perform assessments, diagnosis, triage, health and harm reduction


education/counseling, history taking and other clinical activities according to the HUSSP clinical protocols and College of Nurses of Ontario (CNO) standards of nursing practice


  • Maintain accurate, current client records, chart reviews, data and information and electronically encounter client services in accordance with the Centre’s policies, best practices, and standards of good practice
  • Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services
  • Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses or educational leave)
  • Work as part of an inter-professional team
  • Actively participate in the Centre’s quality improvement and quality assurance plan
  • Engage and provide supervision to volunteers, graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning
  • Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework
  • Provide primary care services on-site, at outreach locations and home visits as appropriate
  • Perform assessments on skin and soft tissue infections and either refer to NP for wound care or perform minor wound care as needed


Qualifications


  • Contribute to knowledge exchange products
  • Demonstrated ability to de-escalate conflict as an asset
  • Confidence in asserting and maintaining strong boundaries
  • Experience with and sensitivity to issues impacting on populations in the downtown core which may include immigrants and refugees, isolated seniors, people living in poverty, people living with mental illness, people who use drugs and/or people who are homeless or at risk of being homeless
  • Experience in program development, implementation monitoring and evaluation an asset
  • Bachelor of Science in Nursing degree
  • Knowledge of harm reduction principles, injection drug use and associated health issues
  • Demonstrated ability to work with multi-disciplinary teams
  • Experience in primary care or street/outreach nursing an asset
  • Respects and values the diversity of communities and individuals
  • Proficiency in the use of electronic medical records
  • A current police record check
  • Current registration with the College of Nurses of Ontario
  • Experience in a community setting or combination of community, hospital, or public health setting
  • Strong oral and written communication skills
