Job Description
Job Summary


We are currently seeking a Registered Dietitian to work within Greater Toronto Area. As a Registered Dietitian, you will work within our talented interdisciplinary team, in our in-home program. Must be willing to travel around the Central LHIN area and sometimes outside, and ensures compliance with internal and external standards of practice.


Duties And Responsibilities


  • Network with internal and external stakeholders to promote new and existing programs
  • Provide assessment and treatment to a wide variety of clients with varying needs
  • Comply with continuous quality improvement initiatives of Bayshore Home Health
  • Liaise with Provincial regulating body as required.
  • Provide nutrition service to clients that incorporates all components of the therapy process model
  • Perform regular, timely charting, reporting and communication
  • Attend staff meetings and case conference meetings, as required
  • Work closely with the team and other resources to develop new programs to meet identified client needs
  • Record workload measurement data
  • Participate in qualityactivities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Work as part of a multidisciplinary team to deliver excellent client outcomes
  • Liaise with other Case Managers,funders or Government & hospital programsas required.
  • Adhere to Bayshore Policies and Procedures.
  • Complete other tasks as requested.
  • Developing a flexible, responsive system to meet changing and competing demands
  • Participates in proactiveHealth& Safetyactivities while performing all duties. Is responsible to notify immediate Supervisor of any Health& Safetyrisks or concerns
  • Provide assessments, interventions, client education, and consultation to home and school, as required
  • Demonstrate knowledge of Standards of Practiceguidelines of Provincial regulating body.
  • Actively participates in and demonstrates support for continuous quality improvement initiatives
  • Positive work and attendance performance essential
  • Maintains confidentiality of clientand corporate information and discusses same only with appropriate Bayshore personnel.

  • Submit all clinical client reports within the pre-determined timeframes

  • Bring knowledge and/or experience with Total Parenteral Nutrition (TPN) (an asset)
  • Thrive in a fast-paced, changing environment
  • Able to remain calm under pressure; role model composure
  • Be able to work both independently and as part of a team, and to facilitate our clients’ needs by exceeding their goals.Consult, collaborate and communicate with members of the client’s treatment team
  • Be comfortable working collaboratively within an interdisciplinary team
  • Maintains positive and collaborative working relationships within the interdisciplinary health care team
  • Have excellent time management and organizational skills and be able to work Independently
  • Return completed charts promptly after client is discharged from services
  • Ability to assess risk, and effectively utilize staff resources throughout all sites to support patient acuity and skill level of staff members while considering the global needs of all patients
  • Have strong skills in critical thinking, decision-making and analysis
  • Hold a valid driver’s licence and have access to a reliable vehicle.
  • Document and report all tasks/information according to Bayshore policy
  • Be dedicated to providing the highest quality of client care
  • Communicate with vendors, or other relevant individuals following consent and privacy legislation
  • Demonstrated ability to be self-directed and thrive in a dynamic environment
