Return of Service Grant up to $5000 for a 6 month commitment


CarePartners is one of Ontario’s largest, most reputable and well-established accredited home health care providers, providing nursing, personal support, home support, palliative, therapy and rehabilitation support services for patients of all ages across urban and rural Ontario. We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 21 nursing clinics.


We continuously innovate to meet patient needs with specialized (award-winning) care programs and technology, including virtual care and AI.


This role is part of our Intensive Personal Support Services (IPSS) and supports patients coming out of hospital while they are in transition to home care. The patients seen are typically complex and the care plan decreases in frequency as they transition to community PSW care.


We are looking for Personal Support Workers (PSWs) to help us bring patient and family-centered care to in patients in the Elgin area. Are you someone we should meet?


Hours of work are 0800-2000 (12-hour shifts), 37.5 hours per week

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
