Work Hours:


14 hours/week, Saturday-Sunday, 7:30 am – 3:30 pm & 3:00 pm - 11:00 pm


Work Setting:


Exclusively Onsite, St. Hilda’s Location


Salary:


PSW - $19.36/hour $3/hour wage enhancement

Program Overview – Supportive Housing, Community Care


The Supportive Housing Program provides housing for seniors who have been street involved, experiencing mental health and/or addictions issues. The program provides onsite psychosocial, primary care supports and services including: intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.


What You Will Do


  • Practice a client-centered and culturally competent approach.
  • Ensure the anti-racism objectives and community development principles of the organization are achieved within the program.
  • Assist clients with activities of daily living; feeding, transfers, bathing, skin care, oral hygiene, toileting, and continence care.
  • Apply proper food handling practices in accordance with the Unit’s policy.
  • Continuously observe clients and their environments, and report and document unsafe conditions and behavioural, physical, mental and/or cognitive changes to the Supervisor.
  • Assist clients with instrumental activities of daily living; light housekeeping duties, laundry, meal preparation, and bulk shopping.
  • Communicate and collaborate with caregivers/stakeholders to determine preferred approaches to care.
  • Perform controlled acts which have been delegated by a Regulated Health Professional, and in accordance with the Unit’s policy.
  • Complete and maintain progress notes in client profile binder and the communication log.
  • Ensure there are sufficient program supplies, forms and equipment available; report to Supervisor when supplies are low.
  • Respond professionally to responsive behaviours; if unable to deal with behaviours successfully, report to Supervisor and work with various professional and resource persons as required to support the client.
  • Perform non-controlled acts assigned by the Supervisor, which have been taught by a Regulated Health Professional (or Supervisor), and in accordance with the Unit’s policy.
  • Provide medication assistance or reminders in accordance with the Unit’s Medication Assistance Policy.


What You Bring To The Team


  • NACC Certificate.
  • 2+ years’ experience working with older adults with mental health, addictions issues and street involvement.
  • Personal Support Worker Certificate.
  • Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption).


What Will Set You Apart


  • Able to do frequent light to moderate lifting, walking, bending, stretching and pulling.
  • Knowledge of behavioural management techniques and approaches.
  • Able to work effectively within a team.
  • First Aid and CPR certification is considered an asset.
  • Good communication skills: verbal, written, listening, and reporting.
  • Food safety handling skills.
  • Ability to interact effectively with high risk, marginalized, and vulnerable individuals.
  • Able to work in a second-hand smoke/pet environment.
  • Demonstrated ability and experience dealing with aggressive behaviours and effective de-escalation and boundary setting skills.
  • Understanding of how to work with clients to ensure their ADLs and iADLs are met.
  • Skills in keeping personal and program environment hygiene.
  • Able to be flexible and demonstrate good judgment in assessing clients’ needs.
  • Knowledge of gerontology issues, health care, and social service systems.
