Job Description
1.0 FTE (35 hours/week) – 2 Positions
Hamilton Urban Core Community Health Centre (HUCCHC) is a thriving inner city health centre and a leader in the provision of high-quality primary health services and health promotion programs. Our comprehensive health services are delivered through a dynamic team of multidisciplinary, inter-professional health providers. As an inner-city health centre our client populations include individuals and families with chronic and complex health and social conditions, many of which are living in poverty.
General Responsibilities: We are seeking Nurse Practitioners (NP) to join our Hamilton Urban Safer Supply Program (HUSSP). The program provides clients using street-based drug supply, who are at high risk of overdose in the community and of medical complications due to drug use, a regulated supply of prescription through the NP in an effort to reduce overdose and overdose deaths. HUSSP is a multidisciplinary team that provides primary care and wrap around social supports to eligible safe supply clients. The team consists of Registered Nurses, Case Manager, Health Navigator, Cultural Ambassador and Peer Mentor.
The NP will work within a harm reduction framework to provide assessment and treatment functions for HUSSP clients and work with the broader team in providing administrative and evaluation support for the program.
Specific Responsibilities
  • Liaise with other providers including various HUCCHC teams, shelter operators, clinical partners and other supportive services in community
  • Contribute towards ongoing review of HUSSP clinical protocols
  • Contribute to knowledge exchange products
  • Engage and provide supervision to volunteers, graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning
  • Contribute to chart documentation, participate in chart reviews, case conferences, and make referrals
  • Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities for HUSSP clients
  • Conduct in-depth and focused assessments based on client condition and acuity at intake and on an ongoing basis; provide health and harm reduction education as needed
  • Prescribe safer supply medications as per protocols
  • Charting client assessments, procedures, follow as per College of Nurses of Ontario (CNO) guidelines
  • Case coordination with HUSSP team and clients
  • Engage in collaborative community relationships to facilitate patient access to services
Committed to anti-oppression, harm reduction, cultural awareness and responsiveness in working with diverse populations
Qualifications
  • Proficiency in the use of electronic medical records
  • Current registration with the College of Nurses of Ontario
  • A current police record check
  • Experience in a community setting or combination of community, hospital, or public health settings
  • Knowledge of harm reduction principles, injection drug use and associated health issues
  • Experience with and sensitivity to issues impacting on populations in the downtown core which may include immigrants and refugees, isolated seniors, people living in poverty, people living with mental illness, people who use drugs and/or people who are homeless or at risk of being homeless
  • BScN with Nurse Practitioner certificate and registered with the Ontario College of Nurses in Extended Class
  • Demonstrated ability to de-escalate conflict an asset
  • Demonstrated ability to work with multi-disciplinary teams
  • Confidence in asserting and maintaining strong boundaries
  • Strong oral and written communication skills
  • Knowledge of community services and resources; strong advocacy skills
  • Strong understanding of and commitment to harm reduction, trauma-informed, and anti-oppression practices
