Patient Assessment: Conducting thorough patient assessments to identify their medical needs and work with their physicain to develop a care plan.
Coordination of Care: Coordinating care with physicians, pharmacists, and other healthcare providers to ensure that patients receive appropriate medication and therapy.
Documentation: Maintaining accurate and up-to-date patient records and documentation of care plans, interventions, and outcomes.
Preparation: preparing medications for administration by the physician, prepping patient for on-site injections for therapy and assisting with medication administration
Data Analysis and Reporting: Analyzing data and generating reports to evaluate the effectiveness of care plans and identify areas for improvement.
Liaise and advocate with third-party providers and provincial governments as necessary to determine coverage options
