CCPN $25000 Grant for a 2-year Full-Time Commitment


CarePartners is one of Ontario's largest, most reputable and well-established accredited home health care providers, providing nursing, personal support, home support, palliative, therapy and rehabilitation support services for patients of all ages across urban and rural Ontario. We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 21 nursing clinics.


We continuously innovate to meet patient needs with specialized (award-winning) care programs and technology, including virtual care and AI.


We are looking for a Nurse Specialized in Wound, Ostomy & Continence (NSWOC) for the Woodstock area. The NSWOC will provide wound and ostomy consultation for patients in the clinic and community. If you enjoy caring for a broad range of clients with a variety of wound etiologies and wound care modalities this is the job for you. In addition to make full use of your wound and ostomy care knowledge, each day you will use your skills in promoting self-management by patients and leading the interdisciplinary team to achieve optimal patient outcomes. Your expertise in the management of wounds and ostomies will be sought out by individual practitioners and the various committees and organizations that guide wound care in the region.


CarePartners will provide you with an in-depth orientation to both the agency and wound/ostomy care in the community; ongoing clinical support including online access to clinical expertise; an iPad and the equipment you need to conduct assessments.


We are looking for Nurses with NSWOC certification who desire great flexibility in their schedules to support patients in their home & community.

  • Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending
  • Monitoring criteria for adjunctive therapy for complex or recalcitrant wounds
  • Providing classifications and indications, appropriate utilization to reduce visit frequency and obtain desired outcomes
  • CPR Certification
  • Robust Employee Assistance Program (EAP) for you and your family members
  • Several years of escalating experience managing wounds and ostomies
  • Excellent English verbal and written communication skills
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Practicing conservative sharp wound debridement (CSWD) and use of other debridement methods
  • Functioning as a member of a nursing team who are accountable for the nursing care it provides
  • To protect our patients, we require a current vulnerable sector check and a clear background check
  • Current immunization record including 2 step TB test and COVID 19 vaccination*
  • Using your knowledge of assessment, prevention and interventions regarding wound infections and ostomy care
  • Supporting our patient’s safety by practicing according to policy and standards
  • Providing edema management including bandaging based on complete lower leg assessment
  • Care for patients with consuming wounds, ostomy, continence, provide lower leg assessment including ABPIs
  • Critically appraising the wound care literature and apply research evidence to practice
  • Using your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the Home and Community Care Support Services (HCCSS) referral and other sources
  • Exceptional interpersonal skills
  • A strong desire and commitment to making a difference in the lives of our patients

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
