Experience working with individuals with Type 1 diabetes and insulin pumps, preferred

Develop a care plan for patients with diabetes (Type 1, 2, GDM, Pre-Diabetes) and collaborate with the multidisciplinary team, as needed

Experience in an acute, hospice, home health setting, or long-term care setting a plus.

One year's LPN experience necessary, preferably two years in an acute care, hospice, LTC or home health setting.

Must be a graduate of an accredited LPN program.

Must have a current North Carolina LPN license.

Recent 2 years experience (within the last 2 years) in critical care nursing

Demonstrated expertise in physical assessment, history taking, patient education, and counselling as these relate to thromboembolic disorders

Demonstrated effective critical thinking, problem solving, and decision-making skills

Experienced in using Patient Care Inquiry and Order Entry modules of Meditech

Demonstrated effective interpersonal, and public relations skills

Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse

Knowledge of mental health and substance use treatment.

Knowledge of psychopharmacology (indications and side-effects).

Counselling and group facilitation skills.

Knowledge of legislation and government policies and procedures that relate to mental health and substance use practices.

Provides follow-up and monitoring of complex psychiatric/medical situations, including substance use/dependence, and referral services where necessary.

Initiates and participates in case conferences/meetings with clients, families, service providers and team members to review and plan care.
