- Conduct on call tasks every 4th to 5th weekend and 1-2 evenings per week.
- Meet with potential clients and family members to establish and update care plans.
- Work with colleagues to coordinate various aspects of client care.
- Review evening or weekend schedule and any trouble spots that might arise.
- Work with clients and their families on the issues that arise to ensure positive resolution.
- Generate weekly, monthly, and quarterly performance reports.
- Communicate with care professionals and clients regarding any scheduling changes.
- Conduct quality assurance visits.
- Complete client and care professional introductions.
- Participate in community events and networking events as necessary.
- Ensure outstanding customer service by scheduling and coordinating care.
- Conduct service inquiries and care consultations.
- Strong administrative and computer skills
- CCA or a professional in a health-related field
- Competitive spirit, capable of working independently
- The flexibility to work in a 24/7 environment where work outside of office hours may be required
- Strong relationship building skills
- Must possess a valid driver's license and a reliable vehicle
- 2+ years of relevant experience
