Job Description

As a motivated, results-oriented professional you will meet daily with new and existing senior clients to conduct in-home care consultations, perform quality assurance visits, and develop/update their care plans.

Responsibilities
  • Conduct on call tasks every 4th to 5th weekend and 1-2 evenings per week.  
  • Meet with potential clients and family members to establish and update care plans. 
  • Work with colleagues to coordinate various aspects of client care. 
  • Review evening or weekend schedule and any trouble spots that might arise.
  • Work with clients and their families on the issues that arise to ensure positive resolution.
  • Generate weekly, monthly, and quarterly performance reports.  
  • Communicate with care professionals and clients regarding any scheduling changes.
  • Conduct quality assurance visits.
  • Complete client and care professional introductions.
  • Participate in community events and networking events as necessary. 
  • Ensure outstanding customer service by scheduling and coordinating care.
  • Conduct service inquiries and care consultations.


Education/Experience Requirements: 


  • Strong administrative and computer skills 
  • CCA or a professional in a health-related field 
  • Competitive spirit, capable of working independently 
  • The flexibility to work in a 24/7 environment where work outside of office hours may be required 
  • Strong relationship building skills 
  • Must possess a valid driver's license and a reliable vehicle 
  • 2+ years of relevant experience
