Three or more years of full-time nursing experience – required.

You possess excellent assessment, documentation, verbal, and communication skills

Minimum of 3 years clinical experience or 2 years’ experience in medical case management

Must possess negotiation skills with insurance coverage and the ability to provide complete and timely case management reports as required

Comprehensive knowledge of worker’s compensation insurance and managed care highly preferred

Develop referrals from case managers, insurance companies and utilization review nurses

Strong oral and written communication skills required

Educational Skills Required (Degree(s), Certification(s), License, etc.)

Clinical skills: documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, care coordination

Minimum of one year related work experience in a healthcare setting required

Knowledge and understanding of urban issues, hospital procedures, community resources

Experience with electronic medical records, MS Word, Excel and Outlook

Works with APN and/or Physician to support the physical health needs of behavioral health consumers.

Maintaining daily charting, monitoring nutritional intake, continuously assessing resident's care requirements

A current nursing license in Prince Edward Island or another Canadian province

Overseeing care team to provide exceptional care to residents

Assessments, Medication management and wound care

Personal Care (bathing, grooming, mobility, incontinence care, in-home assessments, wound care, client care management)
